It sounds unlikely, but a two-year trial across Latin America suggests that culturally tuned lifestyle changes — paired with steady coaching and social support — can produce meaningful gains in thinking and memory for older adults at risk of dementia.

Researchers working on LatAm-FINGERS adapted a multifactor lifestyle program originally tested in the United States to local foods, movement traditions and social settings across 11 countries. The trial enrolled 1,065 participants at 12 sites from Argentina to Mexico, randomly assigning them to either a structured, coach-led program or a more flexible, self-directed version.

What made the structured approach different was simple: ongoing guidance. Participants in the Systematic Lifestyle Intervention attended regular supervised exercise sessions, received personalized nutrition counseling based on a locally adapted MIND-style diet, completed computerized cognitive training, had cardiovascular risk monitoring, and met in groups designed to build social ties and accountability. Those in the Flexible Lifestyle Intervention received general recommendations and four group education meetings over two years — helpful, but far less hands-on.

The results were striking. On a combined measure of overall cognition, people in the structured program improved 55% more than those in the flexible program. Improvements were not limited to one area; memory, executive function and processing speed all showed larger gains under the coach-led model. Short phrase: structure mattered.

The team didn’t simply translate materials into Spanish and Portuguese. Local working groups decided which elements needed to stay consistent and which could be reshaped. Exercise routines included familiar forms like salsa or tango and outdoor group workouts. The nutrition plan incorporated regional staples — avocado, quinoa, açaí, aguaymanto, chia and pumpkin seeds — so recommendations felt doable, affordable and culturally resonant. For participants new to digital tools, extra support was provided so the cognitive training and remote monitoring could actually be used.

Inclusivity was baked into the trial. The sample reflected wide racial, ethnic and socioeconomic diversity, demonstrating that multi-domain lifestyle interventions can benefit communities with differing resources and education levels. That matters because low- and middle-income countries face rising dementia prevalence and limited access to costly medical treatments. Practical, adaptable programs offer a scalable alternative.

Publication in The Lancet and presentation at the Alzheimer’s Association International Conference underline the scientific weight of these findings. They also extend earlier evidence from the U.S. POINTER trial, reinforcing a broader conclusion: targeting several risk factors at once — diet, exercise, cognitive engagement and social connection — appears more effective than addressing any single item in isolation.

There are policy implications. Health systems and community organizations can feasibly deliver culturally adapted, coach-supported programs that strengthen brain health. And for clinicians and families wondering whether lifestyle changes are worth the investment, the data point in a hopeful direction: sustained, structured support amplifies benefit.

So what now? Researchers and public-health planners will need to explore cost, workforce training and ways to integrate these programs with emerging drug therapies. But if a weekly dance, a shared meal of familiar ingredients and a circle of supportive peers can buy more clarity in later life, that seems like a future worth building toward.
