A jar of creatine on a gym shelf and a sterile vial in an oncology lab. Strange bedfellows? Not anymore. Researchers at UCLA report that this common workout supplement does more than bulk muscles — it appears to shore up the very immune cells that orchestrate attacks on tumors.
The team, publishing in iScience, followed a trail of metabolic clues inside tumors and found an unexpected pattern: dendritic cells — the immune system’s scouts and conductors — crank up the machinery that brings creatine into cells when they enter the tumor microenvironment. Curious scientists then removed that creatine gateway and watched those altered dendritic cells falter. They survived less well, failed to fully activate, and were poor at priming killer T cells. The downstream effect: fewer T cells multiplied and fewer inflammatory signals flew, blunting the anti-tumor response.

So what happens when you feed the system creatine instead? In mouse melanoma models, daily creatine injections slowed tumor growth. Tumors harbored more dendritic cells, and those cells were louder in their chemical calls for reinforcements. Metabolic profiling revealed the likely reason: boosted ATP levels inside dendritic cells. Think of creatine as a rechargeable battery for immune sentinels — it helps them keep the lights on and the alarm bells ringing, even while cancer competes fiercely for nutrients.
And this effect wasn’t limited to mice. When the researchers treated human monocyte-derived dendritic cells — the same kind used to manufacture dendritic cell vaccines — creatine enhanced their activation and their ability to stimulate human T cells against a cancer-related target. That raises two intriguing possibilities: adding creatine as a supportive supplement for patients receiving immunotherapy, and using creatine during the lab production of dendritic cell vaccines to make them more potent before infusion.
These insights build on earlier work from the lab showing benefits for killer T cells. Now the picture is wider. Supporting dendritic cells means strengthening the pipeline that recognizes tumors and hands off targets to T cells. In short: it’s not just about arming the soldiers; it’s about improving the command center that sends them into battle.

Important caveats apply. All of the data so far come from cell cultures and animal experiments. That means no clinical advice here. Creatine monohydrate has a long safety record for healthy people when taken at recommended doses, but cancer patients, especially those on treatment, should always consult their care team before adding supplements. The UCLA group plans to collaborate with physicians to design clinical trials and test whether creatine can improve outcomes for people receiving immunotherapy.
Science moves in stages. This study nudges open a door that connects everyday nutrition, cellular metabolism, and cancer immunity. If that door continues to widen in human trials, a routine sports supplement might quietly become part of future immunotherapy strategies.
