Imagine opening a pillbox each morning and adding one small habit that nudges the line between independence and dependence further away. It sounds modest. But a new analysis suggests it may matter.

After three years, older adults taking a daily multivitamin showed modest but significant preservation of everyday function compared with those on placebo.

Researchers sifted data from the large COSMOS trial—more than 16,000 participants aged 60 and up—tracking people who were randomly assigned to take a multivitamin, a cocoa extract supplement, both, or neither. At the three-year mark, those in the multivitamin arm reported better functional health on standardized measures than their placebo counterparts.

Functional health here isn’t an abstract concept. It’s the difference between dressing yourself and needing help. Between climbing stairs and avoiding them. The study used the Kansas City Cardiomyopathy Questionnaire to capture symptom burden and a combined clinical summary score that blends symptoms with physical ability. Those two areas showed the clearest gains.

Why might a multivitamin have this effect? The investigators propose several, overlapping explanations. Micronutrients can support brain function—subtly preserving cognition that helps with planning and coordination. They may also help circulatory health and endurance, reducing fatigue or breathlessness that makes ordinary tasks feel like major exertions. Small physiological boosts can translate into real-world independence.

Not every supplement in the trial produced the same result. The cocoa extract did not change functional scores across the whole study group, though it appeared helpful for participants with congestive heart failure in subgroup analyses. That nuance matters: effects can be specific to population and condition.

The lead presenter, Bayu B. Bekele, PhD, noted that the study’s strength is its emphasis on how people actually function day to day, rather than only counting major disease events. Still, he and co-investigator Yanbin Dong, MD, PhD, caution that multivitamins aren’t a magic bullet. They aren’t a replacement for good nutrition, regular physical activity, or medical care. And anyone thinking of starting a new supplement should discuss it with their clinician.

These findings were unveiled at NUTRITION 2026 and were supported by NIH grant HL157665. They add an intriguing chapter to the conversation about simple, low-risk strategies that could help older adults preserve autonomy as they age. Could a small daily pill become one more tool for staying active and independent? Time—and more targeted trials—will tell.
